Horizons Residential Care Center is a 501(c)(3) nonprofit agency that provides continuous long-term care for children and adults with profound intellectual and developmental disabilities as well as medically complex comorbidities. Horizons’ Intermediate Care Facilities (ICFs) admit our most vulnerable neighbors from all over the State of North Carolina, with the goal of giving the most marginalized sub-sect of the disability community a place to call home. Which is why we created the Horizons Housing Initiative! Acting in response to the COVID-19 pandemic and the anticipated impact of Olmstead v. L.C., Horizons and it’s Board of Directors have decided upon a proactive response to the future challenges that long-term care facilities face. We’re building six new homes on-campus in Rural Hall – to be occupied by up to 36 deserving individuals with severe disabilities and medical comorbidities. These smaller, more individualized homes will support increased independence, improved health outcomes, and provide top-quality fair housing for our neighbors living with severe disabilities. The renovation of our existing building, the Atrium, will include creating a step-down hospital unit for our most medically fragile residents. Designed by stakeholders specifically for individuals with disabilities, these smart homes will serve as the new model for North Carolina (and nationwide) provider agencies.
Phase 1 consists of building two 6-bed homes in Rural Hall to replace Horizons’ outdated and insufficient ‘Arches’ congregate care home in Winston-Salem. The project already possesses significant momentum and funding. Building is underway and the anticipated move in date is Fall 2025. Forsyth County has invested $1.9 million into phase 1 through the American Rescue Plan Act of 2021 (ARPA), and the rest of the funding for phase 1 has been secured through private gifts, foundations, and Horizons’ investments. Funding activities for Phase 2 and Phase 3 are in progress and will be supported through a future capital campaign.
