Horizons Residential Care Center is an Intermediate Care Facility serving Individuals with Intellectual and Developmental Disabilities (IDD), in the Rural Hall and Winston Salem communities, providing 24/7 support and a loving home. We offer services for both children and adults with intellectual and developmental disabilities. Our supports include medical, physical, nutritional, educational, recreational, and personal care supports. A total of 40 people live at Horizons Atrium and Arches homes, which are 15 minutes apart.  We also offer a weekend community-based group respite program for people with disabilities living in the community.

If you are interested in applying for a future opening within our Residential or Respite programs, please click on the application below and complete it.  All applications for admission are reviewed by the Admissions Team, an interdisciplinary team, and if deemed appropriate for services, either begin services or are placed on a waitlist for a future opening.
