Programs and Services

At Horizons we offer high quality services to support individuals with complex medical, physical, and educational support needs.  To provide a holistic quality of life we provide an array of individualized services that are designed to maximize independence and obtain an enviable life.

Residential

Horizons offers residential services for up to 40 individuals. We serve both children and adults with complex support needs and intellectual disabilities.

The Atrium is a 30 bed home that supports both children (up to 22) and adults (minimum 8) with Intermediate Care Facility (ICF) eligibility.

Our second home, the Arches, is home to 10 adults with ICF eligibility. The loving and caring support staff provide 24/7 care for each individual in our two homes.

Within these two homes, we offer 24/7 Nursing and Direct Support Professional care, a specialized school program along the adapted curriculum, recreational opportunities, active treatment, individualized programming, and various therapies (ex., Music Therapy, Equine Therapy, and Hydro Therapy).

Respite

Horizons offers a weekend respite program which offers families in the community a program to enrich the lives of their loved ones. This program is community-based and supports active involvement in the greater community.  Individualized are challenged to explore new opportunities, learn new skills, and develop friendships with members of the community.  This program provides families of children (most ages) and adults with disabilities with a break from the daily routines and challenges in providing continuous home-based support.

Summer Camp

Horizons offers an 8-week inclusive summer camp experience for children 2nd grade to the 8th. Our inclusive summer camp offers campers with opportunities to explore activities and events across the greater Piedmont/Triad region, while developing meaningful social relationships and building upon various strengths and areas of development.

The summer camp includes a combination of on-site and community-based recreational activities (i.e., horseback riding, swimming, hiking, museums, science centers, parks, etc.).
